Data Collection Method/Tool:  Facility registers, project line lists, vaccination campaign tally sheets

Data Source: Vaccine Records, Project data clerk, Health Programme Manager or MEAL officer

Who Collects: Outreach or Facility / H&N Personnel - (If available, data clerk)

Frequency of Collection: Daily / Weekly
Frequency of Reporting: Monthly or Aligned with donor reporting requirements

How to count and aggregate: Count unique doses of vaccine delivered during the calendar year 2023.

If one person receives 3 different doses of the same vaccine in January, June and December 2023, respectively, this counts as three doses delivered.
If one person receives 3 doses in December 2022, March 2023 and July 2023, this counts as 2 doses because only 2 of the doses were administered in 2023.
If 30,000 doses of vaccine are administered during a campaign in June 2023, this counts as 30,000 regardless of the number of individuals vaccinated.
