For example (a complete list of questions is available via links provided):
Has [name] had an illness with a cough at any time in the last 2 weeks?
Has [name] had fast, short, rapid breaths or difficulty breathing at any time in the last 2 weeks?
Was the fast or difficult breathing due to a problem in the chest or to a blocked or runny nose?
Did you seek advice or treatment for the illness from any source?
Where did you seek advice or treatment? Public Sector/Private Medical Sector/NGO Medical Sector/Other source (specify)?

Informal service providers may not be included in this indicator.

To calculate the indicator's value, divide the number of children under age 5 with ARI symptoms in the last 2 weeks for whom advice or treatment was sought from a health facility or provider by the total number of children under age 5 with ARI symptoms in the last 2 weeks.
